Accidental overdoses, some of which have resulted in death, have been blamed on confusing packaging for the powerful painkiller morphine. The accidental overdoses of morphine sulfate oral solutions were mostly attributed to the high potency 100mg/5mL product. In most of these cases, morphine sulfate oral solutions ordered in milligrams (mg) were mistakenly interchanged for milliliters (mL) of the product, resulting in the patient receiving 20 times the prescribed dosage. Reports of abnormal heart rhythms may have been the serious side effect that led to the removal of the popular prescription painkillers Darvocet and Darvon from the U.S. market. But the drugs have also been linked to an increased risk of fatal overdoses, both intentional and accidental. Both Darvocet and Darvon contain the active ingredient propoxyphene, which was approved by the Food and Drug Administration (FDA) more than 50 years ago to treat mild to moderate pain.
