Authorities investigating the crash of a double-decker tour bus that sped out of control and crashed in downtown San Francisco’s Union Square Friday say that brake failure may have been to blame for the crash and that the bus had not been registered or inspected by state regulators as required by law. The crash injured about two dozen people, including five in critical condition who remain hospitalized in San Francisco General Hospital.
